Preschool
Preschool requires these Preschool Application and forms
 
Contact Emelin Tahhan, Preschool Director, (626) 388-5752 with any questions as you look over the application and forms. When ready, fill them out. Fax or mail the completed application and forms to the school. Fax: (909) 792-7292
 
Requirements for Acceptance of New Preschool Students
 
Preschool applicants must:  
  • Be 2 years 9 months by the first day of school
  • Be fully potty trained
  • Provide copies of Birth certificates
  • Provide up-to-date Immunization records
  • Provide current Physical Exam
 
Transitional Kindergarten - 8th Grade

After your application is received, you and your child are invited for an interview with the Principal, to get to know one another, and answer questions. Finally, report cards and the latest test scores are reviewed for your child.  
 
Acceptance
 
Acceptance is based on the family’s ability to:
  • Meet academic admission standards
  • Meet behavioral standards
  • Accept and practice the faith and values that are an integral part of the Catholic School Community
  • Agree to comply with the school philosophy and policies
 
Requirements for Acceptance of New TK-8 Students
 
Transitional Kindergarten applicants must:  
  • Be 4 years of age by September 1st
  • Provide a recommendation from the preschool teacher (if applicable) identifying strengths/readiness
  • Successfully complete the developmental readiness screening
  • Provide copies of Birth and Baptismal certificates (if applicable)
  • Provide up-to-date immunization records
  • Provide current Physical Exam
 
Kindergarten applicants must:  
  • Be 5 years of age by September 1st
  • Provide a recommendation from the preschool teacher (if applicable) identifying strengths/readiness
  • Successfully complete the developmental readiness screening
  • Provide copies of Birth and Baptismal certificates (if applicable)
  • Provide up-to-date immunization records
  • Provide current Physical Exam
 
First-grade applicants must:
  • Be 6 years of age by September 1st
  • Provide a recommendation from the Kindergarten teacher identifying strengths/readiness
  • Be evaluated for readiness in language, motor skills and perception
  • Provide a report card from Kindergarten if transferring from another school
  • Provide copies of Birth and Baptismal certificates (if applicable)
  • Provide up-to-date immunization records
  • Provide verification of a doctor’s physical examination after March 1st of the year of Kindergarten entrance
 
Second, through Eighth-grade applicants must:
  • Be enrolled in good standing at their current school
  • Provide report card and latest test scores if transferring from another school
  • Have their current teacher complete an SHA provided evaluation form
  • Be evaluated for readiness in Language Arts and Mathematics
  • Provide copies of Birth and Baptismal certificates (if applicable)
  • Provide up-to-date immunization records and tuberculosis clearance
  • Provide a copy of their First Communion certificate (grades 3-8)
  • Be transferring from a Catholic or Christian school if starting 8th grade
 
Enrollment and Registration
 
Upon acceptance, parents will be notified by email with instructions and a link to complete enrollment and registration.
